Durable Power of Attorney for Healthcare Decisions: Allows you to appoint someone who can make decisions about you receiving (or not receiving) healthcare if you are incapacitated;
General Durable Power of Attorney: Allows you to appoint someone who can make decisions in all other areas of your life (other than healthcare);
Advanced Directive (Living Will): Gives you the opportunity to state your wishes regarding the life-saving healthcare treatment you want (and do not want) so all family members and care providers are clear on your wishes.

Additionally, the Family Protection Plan PLUS provides these three documents as well as a simple Will to assure that if something happens to you, your estate is accounted for and is distributed according to your wishes.
